Qualifications
- Current and valid LPN license (required)
- Graduate of an accredited nursing program
- Previous clinical or home health experience preferred
- Strong attention to detail and care planning
- Compassionate and professional demeanor
Key Responsibilities
- Administer medications and basic treatments
- Monitor vital signs and report changes to RN/physician
- Assist with wound care and dressing changes
- Support clients with personal care needs
- Educate clients and families on care routines
- Document all care provided
